• Two colored LATEST passport / stamp size photograph of the child and one each of parents.

• Photo Copy of Birth Certificate

• Admission Form

• Self attested copy of Adhar Card of the child

• Self attested copy of blood group of the child (report from authorized lab/certified by MBBS doctor)

• Self attested copy of Adhar Card/voter id card/passport/ration card of both father and mother

• Self attested copy of Caste certificate ( in case of SC/ST/OBC only)

• Photo Copy of Report Card / Mark sheet.(if any)

• Original copy of Transfer Certificate from the school last attended.
